Serial MR image registration and analysis techniques applied to studies of differential cerebral atrophy in late-onset dementia.

Abstract

We have used serial MR image registration and analysis techniques previously developed for studies of cerebral atrophy in early-onset dementia and have applied them to late-onset dementia patients with images acquired using a different scanner and scan sequence. Validation and performance tests showed that with only minor adjustments of key parameters the analysis technique can be applied to scans with dissimilar image characteristics with little change in accuracy. The uncertainty for atrophy measurement was found to be between 1 sigma approximate to 2ml and 4ml depending on the level of care taken during image acquisition. Analysis of registration techniques indicated that 9 degree of freedom registration can be used as an efficient means of correcting for scanner pixel size variations without introducing bias to the measurement of atrophy. The analysis technique was applied to groups of patients with Dementia with Lewy Bodies (DLB), Vascular Dementia (VaD), Alzheimer's Disease (AD) and controls. Mean (+/- 95% confidence limits) annual percentage atrophy rates were found to be: DLB 1.4 +/- 0.7%; AD 2.2 +/- 0.6%; VaD 1.8 +/- 0.6%; controls 0.5% +/- 0.3%. The AD and VaD groups had reduced mean atrophy rates compared to controls, and subjects with DLB had a mean rate lying in between the AD/VaD and control groups but did not differ significantly from either. The analysis method used is shown to be a robust, accurate and transportable tool for studies of atrophy in dementia.
