Evaluating plaque control interventions on gingival lichen planus

Abstract

Aim: To investigate the association in clinical outcome and oral health related quality of life (OHRQoL) following the intervention of a personalised plaque control programme. Methods: 82 patients were recruited to a 20-week randomised controlled trial. The intervention was personalised plaque control comprising powered tooth brushing and interdental cleaning advice. The primary outcome measure was the oral health impact profile (OHIP-49) with secondary outcomes of pain (VAS), plaque index (PI), mucosal disease score (MDS) and cost-effectiveness. Comparison of clinical outcome measures across treatment groups was undertaken using ANOVA, Cohen’s d effect sizes were also calculated. Results: The demographics of the study population were as follows: Mean age 61.4 years +/- 10.9; the ratio of females: males were 67:15. In total 77 patients completed the study. Baseline scores were similar in both groups for OHIP-49 (p > 0.99), VAS (p>0.99) and PI (p>0.93). Difference between groups at baseline in MDS existed but were not statistically significant (p>0.05). Both groups showed improvements in sum OHIP scores; net improvement was significantly greater than in the intervention group at week-4 (p=0.02) and week-20 (p=0.004). Those impacts that occurred ‘fairly often’ or ‘very often’ (FOVO) were significantly fewer, indicating improvement in OHRQoL in the intervention group at week-4 (p<0.05) and week-20 (p<0.05). Clinical improvements in PI and MDS were greater for the intervention group at 4- and 20-weeks (p<0.0001). Conclusion: A personalised plaque control intervention was more effective than control in treating patients with the gingival manifestations of oral lichen planus.
