Frontal Sinus Surgery approaches: An evolution

Abstract

Endoscopic access to the frontal recess has vastly improved over the last two decades in parallel with technological advancement in imaging, intraoperative surgical navigation, instruments, endoscopes and drills. The intranasal endoscopic approach should be the procedure of choice for the vast majority of cases. However, in selected cases an external approach maybe necessary and all surgeons performing frontal sinus surgery should be familiar with this. The aspiring sinus surgeon should have a thorough knowledge of the frontal recess anatomy, aim to preserve recess mucosa and should have a range of suitable instruments at his disposal to achieve his surgical objectives. Despite the armamentarium of frontal sinus approaches, there is however no substitute for careful selection of appropriate surgical candidates.
