Peeling of the ILM from the retinal surface with finesse forceps; The PRECISE study.

Abstract

Alcon/Grieshaber Finesse ‘shark skin’ forceps have been developed and CE marked to specifically improve the ease of picking up the ILM atraumatically during ILM pinch peeling by creating a micropattern on the end of the forceps which increases the ease of ILM grasping during pinch peeling. This should reduce inner retinal trauma during ILM peeling by reducing associated retinal trauma but this is unproven and speculative. It is possible they could increase retinal trauma or only reduce retinal trauma in individual surgeons, dependent on micro-technique. We propose to study a variety of retinal changes following ILM peeling in a pilot masked randomised controlled trial of 66 patients undergoing ILM peeling for idiopathic macular hole with conventional ILM peeling forceps as compared to the new Finesse Forceps. Specifically, intraoperative videos will be used to assess manoeuvres and associated retinal changes during ILM peeling and the relevant images linked to the postoperative imaging as performed by Scupola et al. Three centres will take part with a minimum target of 11 patients. The study will concentrate on objective signs of retinal trauma assessed by masked imagine assessors but will also assess surgeon related factors in a questionnaire as well as functional endpoints including visual acuity and visual fields after surgery.
